Bring Your Pet’s Records
To give your pet the best care from the start, it’s helpful for us to know their history. If your pet has been seen by another veterinarian, please bring or email (rx@dhah.org) the following. We prefer to have your records before your appointment.
- Medical records, including vaccine history, past treatments, and any ongoing care
- A list of any current medications or prescription diets
- Notes on any behavioral or health concerns you’ve noticed recently
If you need help requesting your pet’s medical records, just let us know.
